​Join the Take Control Diabetes program with Dietitian, Gauri Donald and Diabetes Educator, Amy Zheng. We are offering a special after-hours program over the course of 4 months to clarify your confusions and take on the journey of a healthier lifestyle.

Cost: $30 per session with rebate of $19.30 from Medicare

How to register:
Call the Madison Medical Practice Hornsby and book a visit with Gauri or Amy for a 45min session before commencing the program.
Program timings: 5:50pm – 8:00pm
Where to park: We encourage you to park in the Hornsby Westfield shopping Centre which will be open for late night shopping till 10pm.
Take Control Diabetes Program
The Take Control Program will be held one Thursday evening per month at the Hornsby Madison Medical Practice. We envision these evenings to be a fun and engaging and wish to leave you feeling confident in managing your diabetes. This new service is partially funded through Medicare, however, to run a program we will require your commitment for the entirety of the program.
We look forward to having you join us for this rare opportunity. The program will be capped at only 12 participants, so be quick not to miss out.
